1. A copy of the Consular Card* (to apply for a Consular Card, visit the page here)
2. A recent Malagasy birth certificate (less than 3 months old)
3. A residency certificate or an accommodation certificate or an electricity or rent bill (less than 3 months old). The document must imperatively indicate the exact address of the applicant.
4. Employment or school certificate.
5. A copy of the visa or the residence permit.
6. The expired passport or, in case the passport is not expired, a copy of the passport, to be certified.
7. A copy of the expired passport.
8. A marriage certificate for married woman.
9. A Malagasy citizenship certificate for applicant with a foreign sounding name.
10. Passport application form duly filled out.
11. Four (4) recent passport size photos.
12. A letter of proxy on which your signature will be certified by the Embassy. Provide a sample of your signature to the Embassy separately on a white paper.
13. A copy of the National ID card (if available), to be certified.
14. The paperwork must include the name and the phone number of the applicant’s relatives in Madagascar.

For children under 21 years old:

1. A parental authorization from both the father and mother to issue a passport for the child. To be certified.
2. A copy of the National ID card of the parents, to be certified.
3. The Embassy will issue an individual residence certificate for each applicant.

Documents to submit to the Ministry of Foreign Affairs of the Republic of Madagascar

1. All paperwork shall be submitted by the applicant’s relatives in Madagascar to the Ministry of foreign Affairs (“Direction de la Diaspora, porte 132, Ministère des Affaires étrangères, 1 rue Andriamifidy, Anosy, Antananarivo 101”) for validation and submission to Ministry of Public Security.
2. Proof of payment of the passport renewal fee of 190.000 Ar issued by the Ministry of Public Security (Service of Passport) **
3. The applicant’s relatives in Madagascar can pick up the new passport with a proxy letter at the Ministry of Public Security (Service in charge of Passport).
